Below are forms which can be completed ahead of time to speed up your intake. The intake form is only needed one time, as it gives us a starting point for your experience. Complete the form(s), which can be filled out online and then printed, or printed and manually filled out. Bring the completed copy to your appointment, or email it to Nayruslovellc@gmail.com.
Every new client should fill out the Introductory Visit Intake Form.
Only fill out the Child Consent Form if you are the guardian of a minor, aged 13 or up, who would like to experience reiki. They should also fill out a form, separate of yours, as I require permission from all who are receiving the reiki experience.
